Appointment Software

Need a program that will allow users to set an appointment with me but in order to reserve a time, they must make a deposit. The program will have the ability for users to go in and select from available time slots that are not taken by other users. If they cancel within certain time parameters, they will receive their money back, but if they miss appointment they lose deposit. I would have to manually return the deposit if they cancelled within an appropriate time. The program will detect if this is within the proper timeframe. The program will require credit card processing. Program takes visitor to shopping cart to add deposit with their credit card. Program is web-based and will need attractive design. Would be nice if you can do design if not your speciality, will have a web designer do that part but will need to know how information should best be arranged on the appointment page. I will need to receive an e-mail notification that I have an appointment request after user selects time slot. Then I will send back an acceptance or decline. Decline means I will manually return their deposit Acceptance means I will keep the deposit, which will go toward their purchase. I will administer this I suppose through a Mysql database. I would ideally like this program to somehow be active on my desktop and the info could come in from the database to my desktop. So that when an appointment comes in, i get a popup with sound to alert me of the time of the next appointment. Pop-up will have customer's name telephone number and e-mail address as well time of their appointment and how many minutes they have and will stay open until I close. Like a reminder. Is this possible? I have Miva Shopping Cart 4.2 and the confirmation e-mail will have to come from Miva as far as the deposit having been made. Also, capability of waiving deposit if someone making appointment for other than paid services.
