Online Survey and Report

I need a survey tool developed that does the following: 1) individuals answer questions and their responses are saved in a database (MySQL Server). Raters are able to save survey and continue at a later point in time. Raters are sent an email when completed the survey. 2) a report is produced with their responses which: a) summarize responses in rater groupings (eg. if 2 raters clicked Peers then their results are averaged) b) text comments are captured as written ## Deliverables I have included two files: One is the multirater form which is a sample of what needs to be converted to html and attached to the database (it includes 11 categories - the actual will be 5 categories with 5 statements each) The other is the report (which will be produced in pdf with results of rater information). Notes: Scores of 1 will not be factored into averages I will need an admin panel to check status of surveys and to print reports
