Hi,
I am looking for a developer to create a customised sign up form on Shopify. I believe this needs to be done by building an app.
We offer customised pet meals on a subscription basis. During the sign up process, a customer will provide profile information about their dog (breed, gender, age, etc.). The site will then take this information and use an algorithm to calculate the daily number of calories required by the dog, and then calculate pricing depending on the number of calories and the recipes the customer selected.
I have attached an outline of this sign-up process as well as some screenshots of one very similar.
Recurring billing will also need to be set up on the site.
Please look at the attached files and let me know the cost and how long it will take to build.
