Donation Button does not work and new form needed

1. There is a donation button at the right top of the page. This button does not work and we need to make it work.
2. New page is needed, there there will be a form with inputs, suitable with our web page design. There are 5-7 fields and 1 button: Send. When clicked the button, the filled information should be send to 2-3 emails. It should be sent anonimously.
