I'd like the above exact form to dump results into a Google spreadsheet and also send an email when an entry is made.
I do not want to use Google's pre-made Google form though.

The form data will be written using PHP and mail will be sent using PHPMailer. Some minimum validation can be done for the form. A google account will be required to access the spreadsheets. Thanks
