Simple dentist ordering application

I need very simple application coded in PHP using MySQL for my school project, so the PHP code has to be clean, valid and well-commented. Everything has to work with Javascript turned off. It will use sessions The application will be in czech language in the end, so, all texts need to be in a separate php file, I will translate them. The script will have to be integrated to ordinace.josefvacula.com. I need application for dentists, which will have two parts: part for patients: Patients will be able to register, there will be some simple registration form, pacients will enter Name, Address, Country, Phone, Insurance company and Insurance number. After the registration, patients will be able to login and make a non binding appointment. They will be able to make the appointment through some simple form with calendar, where they will be able to choose from available date and time. The available dates and times are Monday 8:15 - 15:30, Tuesday 8:15 - 17:30, Wednesday 8:15 - 14:00, Thursday 8:15 - 17:30, Friday 8:15 - 12:30. Scenario 1 - Patient wants to make an appointment: Patient logs in and clicks on "make appointment" link. Patient chooses reason for the visit from list of predefined medical procedures, which will be editable from admin area. After it is chosen, patient is able to enter note with more details about patient's problem and then patient is able to choose date and time which he wants. He chooses by clicking on available termin in calendar, every medical procedure requires different amount of time, the amount of time is displayed for user above the calendar. Patient clicks on termin he wants and clicks on "make appointment" button. Reservation is done, the time and date he booked becomes unavailable for other users in calendar when they want to make appointment. Scenario 2 - Patient wants to see binding appointments (accepted by doctor, see "part for admin/doctor) Patient logs in and see list of his accepted or denied appointments part for admin/dentist: Dentist will be able to define new medical procedure - it consists of name and time needed for procedure. Dentist can see list of registered patients and list of reservations, which were made by registered patients. Dentist is able to either accept or deny the reservation. If the reservation is denied, the termin becomes available for patients again, if dentist accepts it, the registration becomes binding and patient is notified by e-mail.
