Modify the Fax Cover Sheet Used by Mac OS

I have a USB fax modem on my MacMini server that is shared with all my users. Its slick, but the Cover Page for the fax that Mac creates is pretty bad. I'd like it to be modified. The cover page is created from a file in /usr -> libexec -> fax -> [url removed, login to view]
I have uploaded a copy of that python file.
I've also uploaded a document called "fax cover [url removed, login to view]". It has two pages:
Page 1. This shows the fax cover page that MacOS generates
Page 2. This shows a mockup of what I would like made instead.
The main additions are the image in the header and my contact information on the upper right along with the disclaimer in the footer.
The central area can be very similar to the default that mac makes except for these changes:
1. Do not bother with the From Variable, instead, just hard code it to say "Miller Family Dermatology"
2. The current script has a place for us to enter a message, as shown on page 1 where it says "what does it look like? does it suck?". Before displying the message, please put in a tag "Message", as shown in my example on page 2.
I will need the code annotated so that I can make small changes to the header (phone numbers, address), and modify the text of the footer if needed in the future.
