Parse Word document into database fields

I have a project that requires that a Word document using a template has certain areas assigned with a bookmark. When the document is saved I would like to also save an xml file with the contents from the word document with the bookmarked fields parsed out. I would like a method to send the xml file to a SQL database for inclusion. In summary I need:
1. A MS Word template with bookmarks inserted. I will supply the example form template. Need to insert hidden bookmarks in the .dot file.
2. Be able to save as XML - I believe the MS Word XML format will work
3. A method to take that XML file and automatically append to MySQL
Overview: I have a custom form template. When a user types data into that form, I would like that data that they type to go into discrete database fields. I think the easiest way to do this is to insert hidden bookmarks within the template (correct me if I'm wrong). The file is then saves as XML and is emailed to a pc. I need an application on the other end to take that XML file and append a MySQL database.
