Ebook Cover Design, Theme Design, and Organization

I have 2 e-books. One is 5 pages and the other is 75 pages. They are both in .doc Word format. I need them to be organized into a .pdf file with a cover page and a theme through all pages. The 75 page ebook needs to have a table of contents and a few pictures throughout to help add to the experience of reading it.

I can pay through escrow or PayPal. I am looking at spending less than $50 for the project. Please show your examples of past ebooks that you have designed.
