Need Design of 2 UI Modules

Im looking for a provider experienced in web based UI design to build the css, graphics and html for 2 modules that will be added to a dashboard for a website. Module 1: This module will filter results by using this slider [login to view URL] (I dont need help with integration, i just need you to be aware of the slider being used) We will have data in striped rows that will be loaded via ajax (we will handle the ajax) to the right of the slider depending on the min/max values. I need someone to come up with the UI for the placement, sizing, etc of the slider on the left for the full height of the div to set a 'Score' value - which will be between 50 and 100. Then to the right should be a title bar of 'Results for Score XX' and below it the rows of results in a scrollable div. Each row will have a first name, last name, their score and a 'view' icon of some sort. Module 2: This module will display as a dialog when the 'view' icon is clicked from Module 1. This module will display a first/last name, score in large number and then a set of rows which show how the score was obtained. Id need some type of icon next to the number as a visual cue to represent the quality of the score. The score will always be between 50 and 100 and in some cases 100+. I will need a graphic of some sort to represent each of these conditions for a score: Poor, Fair, Good, Excellent. The main score is determined by an algorithm comparing two values of various smaller scores. For example, in a personality match test. I may have a score of 78 for 'Spontaneity' but the person may have a score of 85 for the same thing. I need there to be a row for 'Spontaneity' which will show a bar of some sort for 'Me' and for 'This User' representing the two values, and also display the value inside of the bar. Something like: Your Score This User Spontaneity: [XXXXXX78X ] [XXXXXXXX85X ] Outgoing: [XXXX65X ] [XXXXXXXXX94X ] .. There will be -varied- amount of rows, anywhere from 5 to 20 in some cases. ---------- The deliverables would be the css,html and graphics for the above. I will provide the website to the winner, but for now you can assume color scheme of simple white background black text with dark grey borders and trim. I do NOT need any help with ajax or javascript or functionality, I just need all the graphics/content to have available for the functional widget we will use.
