Query & Result Code Needed

I have a mySql database with a table called CREW. The Key field in the table is called category and each crew class has a unique category number...ie 1000, 1001, 1002 The fields in the table are labeled ID Category First_Name Last_Name Address City State Zip Phone Cellular Union Currently Working email website comments I have a html Form page with check boxes like this # Search For Crew When I submit this page I want it to query the database and list all of the names and their info under the categorys checked on the form page. I want them listed in the following manner, 10 records to a page; Results //first record - first category// First Name Last Name Address City, State Zip Phone Cell Phone eMail Website Union Currently Employed Comments //second record or first record next category// First Name Last Name Address City, State Zip Phone Cell Phone eMail Website Union Currently Employed Comments ecetera, ecetera until no more records from the categories checked on the form exsist. It seems no matter what I tried I can't get this right. Need to get this done for my sanity. It is a vanity project I am doing for my wife. I have no problem with the script to access the database and table, only getting the form to request the category (s) and then output it in the manner above. ## Deliverables 1) All deliverables will be considered "work made for hire" under U.S. Copyright law.
