PHP/MySQL Web Form for use on iPhone Safari

I need someone to write a template web form which I will be able to use to update a MySQL database using my iPhone. The process will go like this... 1. I will access my website through the iPhone Safari browser using an address such as this [login to view URL] Where Key=xxxx identifies a unique record in my MySQL database. 2. If record xxxx exists, then automatically open the form with all of the corresponding data for record xxxx so I can edit/update it 3. If record xxxx doesn't exist, then open up an empty form that I can fill in, update, and save as record 1234. This web form will be used primarily through an iPhone using the Safari browser, so I need the form to be optimized for use with the iPhone. The form should include examples of all different types of inputs especially text, drop-down, radio, checkbox, and date. 2 different date/time fields should be included, one which auto-populates based on today's date and time, and another one which populates based off of user input (ideally using some sort of calendar type of device). Ideally, as much as you can use iPhone shortcuts functions, the better. i.e. I've heard of numeric form fields opening up the numpad instead of the keyboard. The php/html should be crafted in a way that I (with a small knowledge of php and html) will be able to easily modify it to create my own unique forms in the future on an as-needed basis. This should be a fairly straightforward project, and currently, isn't for any particular data set, so it needs to be generic and easily modifiable.
