Hi I did the same project which you need using ESP-01 Module. I uploaded the Pic of project on my profile you can see the small hardware........... Basically what it does is, on power on it goes in to STA Mode and obtain IP from router and then start posting requests to a Things Speak server which then extract the data from Post request and then plots it on a graph(Actually its a Battery power monitored data that is displayed on the ThingsSpeak Server). A button is connected to GPIO-1 of ESP which when pressed takes it in to AP mode. Now you can access a webpage from ESP by hitting a predefined IP in browser. The page which is displayed on hitting the IP contains the SSID'S of networks which are in its range. Two fields are displayed on the page in which you put the SSID and Password of new network to which you want to connect. Once you input the new SSID and Password this is stored permanently in ESP. If you switch off the power and then switch on it you don't need to input the SSID and Password again it will automatically load the previous settings. I can show you a small demo on it....
