I require a professional designer to convert my Microsoft Office Form (see PB Form Draft pdf attached) into an electronic PDF fillable form. This includes: - Converting to a PDF fillable form - Adding a good, slick, office and professional design to the form. - Must be user friendly and have a office professional theme. I am looking for something similar to the attached NZF. pdf form. The size of the boxes must be adequate for the related information. The first page is the cover page and must be well laid out and designed.
