Create a PHP application that collects survey information and generates GREAT infographics

I need a new PHP application created. It is the third version of the application. Old files will be made available for reference and graphics, but I need a fresh design and fully confident functioning. The simple description is that this application collects survey responses from churches and creates a report utilizing a set of formulas. The church is the customer. The members are the survey-takers. There are variations of the report with differing levels of detail. Often, but not always, a large group of congregations will buy the assessment – in such cases the group need to be identified for specialty reports and the billing needs to be streamlined. In such cases, the group will have one bill, but each individual church will be encouraged to buy upgrades – so they need a payment mechanism. Each participating church has 3 different surveys adding data to the whole: a pastor public survey, a pastor confidential survey, and a standard participant survey. There is one pastor public survey, one pastor confidential survey, and many standard participant surveys (dozens to thousands). The pastor informs us how many different weekly worship services the church has and how they are identified. That information makes the standard participant surveys unique for each church, because we ask which weekly worship service each participant most often attends. The pastor also chooses the start date for the standard participant surveys. Once started, the surveys will be open for 15 days. At midnight on the 15th day, the survey closes and the standard report is emailed to the pastor. To avoid errors or deliberate hacking, each church needs to be assigned a unique code allowing access to their survey. The code should be a book of the Bible and a 4 digit number. Special reports need to be available for purchase up to 365 days after the date of survey completion. The process needs to accommodate discount codes. I need an administrator access panel from which I can see the status of all surveys and manage discounts. Now, the evolution of infographics have left it looking rather sorry. The new version must have GREAT and informative infographics. We can dialogue about the best data sets for different graphical treatments, but it has to look amazing. Some other consultants want to use this new resource. I need the option to drop a third party logo over the top of the ChurchTech logo on each page. Also, I need the capacity to include a third party set of cover pages at the front of the report, before the numbered report pages begin. Phase 1 of this project requires everything working as above to create a basic and advanced report with all billing options (Paypal is fine). Phase 2 will include additional reports and additional languages. The successful coder for Phase 1 will have first option to carry on Phase 2.
