Simple Javascript

I'm needing a programmer to build a simple javascript program that will calculate a final total (in real-time) on my order form based on the payment plan the customer chooses. If you go to my order page at [login to view URL];accid=1. You'll see that want I'm wanting is when someone clicks "Yearly" the "Total" will populate with the yearly + the setup. Additionally, I'd like it to populate the total with the monthly + the setup if the customer selects the monthly payment plan. I'm needing this done in a few hours. The budget for this is $15.
