Build a Fillable Form Application

I am looking for someone to build a simple app that allows for the following: A fillable form to include uploading photos, making comments, check boxes, digitally initial and sign once completed, etc. The owner of the form needs to do the following: - set up users - invite users to a property/form - create groups of users for a property/form - create a custom form based on bed/baths, number/type of rooms, etc (see template attached) - allow digital signatures and initials The User needs to do the following: - set up an account (or get invited) - fill out form, upload photos, digitally initial and sign, etc - multiple users need to be able to access the same form, make edits, sign, upload photos, etc Once the form is completed and signed by everyone, it needs to be emailed to everyone. Stripe needs to be integrated as the payment gateway for customers to sign up. This will needs to be mobile friendly as well. Please see the attached sheet as an example. Once completed, there will be opportunities for ongoing maintenance and improvements going forward.
