search engine + form - repost

I need a software which collect the record of each player or club and help other player to find the club and/or players nearest. This is a form with some info like name, surname, club name, games played, ecc. then we need a software who read all the info collected of each players and put them on a map like google map and help users to search using differents criterias like city, games, etc. the software could be added on the site [login to view URL] I need an admin section wherer admin can set the fields of the form users can record their insert username and password (the should record the 1st time), there is also a disclaimer in the search engine email of user are not visible but the software let other people to connect the users trought the website. Each user can change also their info trought his username and password.
