Web page for Client information and Helpdesk Log a call

Hi We are an IT support company and we have a SQL database that contains client data eg client name, address, contact person, phone number etc etc and a table that contains helpdesk information about support calls that are logged for each of our clients. We require a web site to be developed for clients to log on and perform some basic functions. So the first page would be a simple userid, password form that will check the logon details in the existing Client table. Once logged on the client will see a number of options. These are as follows Client Information - Displays details of the client and allows them to update their details Log a new call - Allows the client to log a new support call which will be inserted into the Helpdesk table View Open Calls - Shows the client any calls that are open in the Helpdesk table View Closed Calls - Allows the client to select a start and end date for when a call was closed. By default the last 30 days calls would be visable. We also need a button where the client can press print and the displayed records are sent to a printer. Contact Details - Displays a read only page of our office contact details eg Office Phone, Fax, Address etc etc and also the name of the engineer that is asigned to their account. We have an excellent undestanding of the existing SQL data structure and can provide you with all the field names. Only two tables will need to be used in this project. The site needs to look professional (please provide examples of previous work) and must run on IIS. The applicant must have good .NET and SQL skills.
