Custom Mailchimp single opt-in form

Simply we just need a form that we can use with our Mailchimp account that removes the "confirmation" email, and adds the details directly to our list once the user clicks. We need the "double opt-in" section removed. We want the form to look like the standard Mailchimp form aswell, so use the same CSS. Please provide any examples of work you have done on Mailchimp.
