57510 Guestbook script

Need a simple guestbook script that will not allow the posting to be shown until I have approved the posting. If I do not approve then it is deleted. If I do approve, then it is added to the page. Pagination would be good, but not a must. The person who enters the info would need to put name, email address, location and message. Smileys would be nice, but not a must. I need to be able to incorporate this into my own design, maybe using an include or something like that. I would prefer CSV database, but will accept MySql if necessary.
