PHP processes

Really Seeking someone in USA. thanks. I'm needing a very experienced PHP programmer to assist with several upcoming tasks.. could turn into more consistent work for sure. I'm seeking a freelancer with very competitive pricing and someone who is NOT a beginner with PHP as we have some complex processes already in place on some of our sites that we work with ongoing. I have a php guy that already works on our existing complex sites, but I need more help. This bid request and first project is to create a php form with 10 fields (to be provided) that will submit info into database tables some tables already created (I have been entering info manually up until this point). If user is already in database, the user must see a message that asks them if they would like to update their account and be able to update his information. Once form is submitted, I will also need the entry emailed to me. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows? (depending on the nature? of the deliverables): a)? For web sites or? other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software? installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
