65264 Simple form/MySQL

I need a very simple script that does the following. 1. User fills out information (first name, last name, address, city, state, zip, phone number, e-mail). 2. They click on a Submit button and are redirected to a Thank You page (which I specify in my control panel). 3. All of the information is stored in a MySQL database. 4. I can export this information from my control panel into a .csv file.
