Form Creation - Contact Forms

I am looking for someone to create simple contact us forms. The form should include these fields: Name Email Phone Address Comments Box Drop Down Menu with 2 or 3 options Upon submission, the code should be able to be put into a website and possibly carry over a variable such as their name, but is not required. The forms should be written to be as secure as possible and not open security holes on the server. The server is a standard Cpanel based server so php may be utilized. I may also want to have image verification implemented with 3 or 4 digits to keep spider bots out. I know that freelance solutions exist and can even suggest one. The script should allow the ability to email multiple recipients. Can a form be created as a .html? I currently have had them created in .php but am curious to know since I want to add some snippets of tracking code to the thank you page. These are simple forms and I am not looking to spend a lot because I create them myself when I have time, but am looking for someone to create them randomly when I need them. If this project goes well, I will inquire for other programming related work.
