Needing a small simple laravel project put together. We are wanting simple 3 page app built. The app will be used to score and track the quality of call center agent’s calls. This app will be integrated into a larger system so we don’t need auth or much other than the 3 pages we are requesting. We’re looking for a simple layout built with Bootstrap so we can integrate it into our current templates easy. Nothing fancy necessary. Page 1 - Score Call This page will be a form used by the Quality Assurance Agents to score a call. Fields we are needing are as follows: QAed by (string) Agent being scored (string) Phone Number of Call (string) Quality Score (int) Competency Score (int) Pay-period Rev per call (double) Campiagn (dropdown options (CA, EDU)) Comments (long text) All fields but pay-period rev per call, are required. After post the user should be brought back to the form ready to perform a new entry. Page 2 - Score Report This page will be used to generate a report of all the scores entered on the first page grouped by the Agent being scored. Options for the report are as follows: Date/Time Range (down to hour) Campaign (drop down options (CA, EDU)) The report should default to load the current day’s entries. Results should be displayed in a data table that is sortable by all fields. Only fields required to display on the report are as follows: Agent Being Scored Quality Score (Average of all scores for period) Competency Score (Average of all scores for period) Pay period Rev per call (Average of scores with value ignore scores without an entered value) The field containing the Agent Being Scored should link to a 3rd page that will output all the entries that make up the current report. (ie if report is done for month, the link should open the agent report for a month) Page 3 - Agent Report This report will show all the entries that comprise the score agents score. Report can be ran individually with the following options: Date/Time Range (down to hour) Agent To Be Scored (Populated with all Agent’s to be Scored that have a score) The report should give the same aggregate info their entry on the on the Score Report plus a data-table containing all the scoring data for the provided date range, and Agent to Score. Fields for the table should be as follows: Phone Number Quality Score Competency Score Pay Period Rev Per call QAed by Comments
