293085 Event Ticket Creator

I want an event ticket creation script. NOT a ticket selling system, but simply a form that anyone can fill out containing their event's information and how many tickets they want to print. They click submit, and the system creates that number of tickets for the person to print out with their event's information on it and a unique barcode number on each ticket. Along with the tickets the system would also create a simple sheet listing all the barcodes and ticket numbers on it with a check box next to each code, so it can be used as a check sheet when collecting the tickets for an event. I prefer that cool ajax effects are applied if possible, it give a more web 2.0 effect. But, I don't want you to add much css, because I want to be able to easily add this script seamlessly into my website using my site's css. I would like to install the script myself, so it needs to be in php, and simply be a folder containing the files that I can upload to my website. Then using a php include line, I can easily add this ticket creation form to any page on my site. The form/ticket should have the following info: Event Name Venue Name City Country State (if applicable) Date Time Cost Description/Directions The ticket size should be about the same width as a regular sheet of paper , maybe a little smaller, so about 6 or 7 inches wide, and I would like the person to be able to print 3 tickets per standard piece of paper. So three long rectangular looking tickets per page. With dotted lines between each one to make it easier to cut up the tickets for selling. When the person fills out the form, selects the number of tickets he/she wants to create and print and hits submit, then I want the system to bring up a new page showing all the tickets and a checklist at the end for them to print. I don't want anything stored on my site if possible. So if it is possible I want the script to create the tickets, bring them up on a page for the person to print, and then once they close that browser window, that's it the tickets are gone. They'll just have to re-submit the form to make new tickets. In short this is just a super simple event ticket creating script for people that want to sell their own tickets for a small event. The visitor goes to a page on my site, fills out the form, it creates the tickets and a check list, they print them, and that's it. Let me know if you have any questions.
