HTML Form Processing

I have an html form which can be viewed here [login to view URL] which uses javascript to calculate the totals at the bottom of the page. I would it to happen that once the user clicks on "Place Order" they are taken to a page where the same data is displayed without the input boxes and the user can either "confirm order" or "change order" by going back and editing the form. Once they have clicked on "confirm order" I would like for the form data to be sent in a html email (in the same layout as the original form - minus the header) to both the user and the siteowner. Also I would like the invoice number to be generated and unique each time the form is used.
