New functionality to add to existing site (Phone number / Email reveal and log)

New functionality to add to existing site: New PHP page that looks like the attached image, code available to use as a template. On this page - show show a telephone number and or email address and log the request. - Log this request in a new table which records the user_id who requested the number, the car_id, the date and time, the number and the email address shown. I need PHP code to display "Call" or "Email" or both if the data is available in the table registration_consumer for a given car. For testing it needs to work on an existing page [login to view URL] which is mysql/php based. Summary: New table for logging New PHP code do display "Call" or "Email" links if data available New PHP page to display details and record action. Clean commented code, and install instructions after testing.
