PHP development hourly work - MVC / CRUD

We're looking for a good PHP programmer to further develop an internal CRM/order management system. We are currently using this system- it is functional, and the database structure has been defined already. It's uses standard MVC concepts. Most of the models required have already been developed. What's missing is basically some simple CRUD controllers and views- present a form to collect new "user" information, call the ->insert() method of the model in your new controller method to save the data. I will provide a specific set of tasks that I want accomplished (example: create a view, and implement controller methods to both CREATE and UPDATE an "application" record), you can give me a quote on each task, and we can go from there. If we work well together, this will be an ongoing project- there's probably a total of 1000 hours of work, spread over many, many weeks. You should have an understanding of MVC, PHP, and MySQL. PLEASE PROVIDE SAMPLE CODE SO THAT I CAN SEE THAT YOU'RE A DECENT PROGRAMMER. You will not be considered if I don't have some sample code that I can review to make sure you know what you're doing.
