Create CRUD with login & custom code number

I want to create CRUD with login & custom code number with specification like this: 1. Login with username, password and group 2. CRUD system with field like this a. select input (can be B, S, A, PP) b. name (text) c. input number (number) d. date (date) e. code (this code generate when inserted) 3. view mode must contain pagination and custom search 4. Automatic generate code when inserted with criteria like this: 06724/LABEL/0017/L/B/XII/2015 Code separate by "/" 06724 -> running number with 5 character LABEL -> just add "LABEL" 0017 -> get from input number with 4 character L -> group from login page (session) B -> get from select input (can be B, S, A, PP) XII -> month in roman format from input 2015 -> year from input You must use PHP, CodeIgniter, MySQL
