Build a database of sleep physicians -- 2

DSCA is a service provider in the sleep apnea industry. We offer telehealth and remote patient management capability for sleep physicians to manage their patients treatment online and remotely. We are looking to build a data base of sleep physicians that could benefit from our services initially in 11 states in the USA. We need a freelancer to use internet search engines or any other method to identify sleep physicians in urban centers in these states with a population of more than 250,000 people. The data points required are the sleep physician's name, clinic address, email address, phone number(s) and fax number which will need to be entered it into an online spreadsheet. This freelancer will be computer and excel literate with tenacity to dig for details that may not be apparent on first pass. Ability to take feedback on the job to optimize the outcome of the database build. If this database build is successful the other 39 states will also need to be built.
